has been seeking affordable health solutions on behalf of its employer members in Colorado for more than a decade. CBGH develops and implements projects on a community and state-wide basis to decrease individual employer trends in healthcare spending.

Bridges to Excellence or  BTE is a national pay for performance initiative designed to increase quality and value in health care through the reward and recognition of high performing physicians. The focus of the BTE initiative centers on:
 
• Diabetes care, cardiac care, and physician office information technology             
• Selected markets where leadership brings payers to the program           
• Program costs are paid by participating employers and health plans
   

Concept behind Bridges to Excellence:
Attaining population based, high quality healthcare according to evidence-based medicine guidelines
Employers are interested in achieving cost savings for patients seeing high-performing physicians
Willingness to share portion of projected cost savings per patient to physicians as reward for health care excellence
Rewards to top physicians act as incentives for other physicians to change their practice patterns and improve performancePhysicians receive rewards, employers realize cost savings, and consumers’ health is improved


How does the program work?

Physicians voluntarily decide to participate
Physicians request a copy of the National Committee for Quality Assurance (NCQA) electronic instructions and guide to measurement
Physicians create a sample of all eligible patients (ie, patients with diabetes or patients with cardiovascular disease) and enter them into the NCQA spreadsheet
When the sample size is reached, and care processes and outcomes meet or exceed norms at the national 75th percentile of practice patterns, the data is submitted to NCQA


Where is the program available?
The BTE program is available along the entire Front Range of Colorado including both the diabetes and cardiac care programs.
For 2007, 13 physicians have been recognized and $14,700 in rewards has been paid.

• Centura Health** • Colorado Springs Utilities Penrose-St. Francis Hospital
• City of Colorado Springs • El Paso County* • Public Employees’ Retirement Association**
• Colorado College* • Intel* • University of Colorado**
• Colorado Springs School District #11 • Memorial Health System  
*From January 1, 2006 to December 31, 2007
** New as of January 1, 2008
 
 
Participating health plans have signed contracts: Anthem (2007), CIGNA (2007), Great West (2006), United (2007) and Rocky Mountain Health Plans (2006) for the diabetes program in Colorado Springs, and are looking to expand geographically and to add cardiovascular in 2008.
